The Technology Lab and Maker Space are specialty rooms. They are available to the public on an equitable basis, regardless of the beliefs or affiliations of individuals or groups requesting their use. Use of the the Library’s spaces does not constitute an endorsement of an organization’s beliefs or policies.
The rooms are only available during the hours the Library is open to the public and can only be booked quarterly after the release of the Library newsletter. (Library programs have priority booking over non-Library programs).
Meetings must be free. Equipment use in the Maker Space is restricted to those who have successfully completed a safety course with the Library staff.
Regular bookings (weekly/monthly) are not guaranteed because of the demand for the space.
Cleanup is the responsibility of the organization/party booking the room. Failure to do so can result in fines.
MAKERSPACE ACKNOWLEDGEMENT OF RISK & RELEASE FORM
THE MAKERSPACE at the Charlotte and William Bloomberg Medford Public Library (CWBMPL) is an innovative, creative, collaborative, and educational space with access to advanced software, emerging technologies, and coworking spaces.
In consideration for participation in the activities of CWBMPL’s MAKERSPACE, Participant agrees to the following waiver and release:
- Assumption of Risk.
I acknowledge that there are inherent risks in the use of tools, equipment, devices, and hazardous materials of any kind commonly used in electronics construction, fabrication, software design and other technology related activities and that my participation in such activities and/or use of such tools may result in injury, illness, death, or damage to me or my personal property. Further, such risks and dangers may be caused by myself or Guests or other Participants. Finally, such injury, illness, death or damage to me or my personal property, may arise from foreseeable or unforeseeable causes. I hereby fully assume all such risks and any others which arise because of my use of THE MAKERSPACE at CWBMPL.
- Release of Liability.
I waive and release all claims against THE MAKERSPACE/CWBMPL, its officers, agents, volunteers, board members and assigns from any and all liability whatsoever arising out of any damage, loss or injury to me or my property while participating in any activity on the premises of CWBMPL and/or using any equipment owned by CWBMPL or another member or guest of CWBMPL regardless of whether such loss or injury is as a result of negligence of CWBMPL or its members, guests, agents, volunteers and assigns, or is a result of some other cause.
- Covenant Not to Sue.
I agree that I will not, under any circumstances, initiate any legal action against CWBMPL, its members, volunteers, agents and assigns, nor will I assist in the prosecution of any such legal action filed by another, arising out of an injury to the person or property of myself while participating in the activities contemplated by membership in CWBMPL. This covenant extends to include the heirs, executors, administrators or assigns of Participant.
- Damages.
I agree to pay for any damage sustained to CWBMPL property as a result of participation in activities at THE MAKERSPACE /CWBMPL. By my signature below, I acknowledge that I have carefully read this Agreement in its entirety and understand it, and I voluntarily agree to all statements and provisions of this Agreement.
I am sufficiently informed about the Makerspace Activities and Risks involved to decide whether to sign this Agreement.
By my signature [above], I acknowledge that I am a legal guardian or other person legally responsible for the Minor User with authority to sign this Agreement for and on behalf of the Minor User. I have carefully read this Agreement in its entirety and understand it, and I voluntarily agree to all statements and provisions of this Agreement, for and on behalf of the Minor User. I am sufficiently informed about the Makerspace Activities and Risks involved to decide whether to sign this Agreement for the Minor User. I attest that I am eighteen (18) years of age or older.
